Community Health Choices (CHC) provides essential managed care in Pennsylvania to residents in need of health services. A Medicaid Expansion initiative, CHC aims to simplify the healthcare process and ensure that Pennsylvanians have greater access to the insurance they need.

They work with the person and their families to identify desired life goals and find service providers to achieve these outcomes. Services are typically funded through one of three Pennsylvania waivers: the Consolidated Waiver, the Community Living Waiver, or the Personal/Family Directed Supports Waiver.

 

Eligibility for intellectual disability services requires a diagnosis based on objective standardized testing, indicating significant impairments in daily functioning. ECCM's intake department assists individuals in navigating these guidelines, collecting necessary documentation, and determining eligibility.

 

Support Services for Individuals with Autism

ECCM provides support coordination to help individuals with autism realize their dreams of independence. This includes developing and maintaining an Individual Support Plan (ISP), identifying needed services, locating qualified providers, coordinating funding, and monitoring service delivery.

 

Eligibility for autism support coordination requires a diagnosis of Autism Spectrum Disorder (ASD) based on standardized testing, with evidence that the diagnosis occurred before the individual's 22nd birthday.

Prioritization is given to households with significant financial hardship for up to 18 months of assistance.
